The Blazer Education Foundation provides grant funding to support innovative programs, projects, and opportunities that enhance the educational experience of Daniel Boone Area School District students. Employees of the Daniel Boone Area School District are eligible to apply for grant funding.

Please complete the grant request form below with information about your proposed program or project. Before beginning, download the Grant Budget Template, which must be completed and uploaded as part of your submission.

Please provide enough detail for the Foundation to understand the purpose of your request, how funds will be used, and the anticipated benefit to students. All requests will be reviewed by the Blazer Education Foundation, and applicants will be notified following the review process.

Program Evaluation & Grant Recipient Agreement

By accepting funding from the Blazer Education Foundation, I agree to support the evaluation and promotion of the funded program or project. If awarded a grant, I agree to:

Provide a follow-up on the implementation and success of the program, including outcomes, impact, and other information requested by the Foundation.
Participate in reasonable promotional and communication efforts related to the grant, including photos, social media, website content, or other opportunities to highlight the program and its impact.
Provide all documentation necessary to support grant expenditures, including purchase orders, receipts, invoices, or other requested financial records.
Use awarded funds only for the purposes outlined in the approved grant application and notify the Foundation of any significant changes to the project or intended use of funds.
Respond to reasonable follow-up requests from the Foundation related to the implementation, evaluation, or promotion of the funded program.
